Jacob Tai Nathaniel Bentley, 2 months of age, passed away Sunday, April 18, 2004 in Corbin.
He is survived by his Mother Susan Lewis; one Sister Chelsea Lewis; Grandmother Melanie Baker; Great-Grandmother Susie Rachel Bentley; four Aunts, Pavielle Baker, Tiffany, Baker, Ashley Keisha Baker, and Victoria Janelle Baker; and by one Uncle Joshua Paul Levi Baker.
Funeral services for Jacob will be at 2:00 pm Wednesday, April 21, 2004 at the Vankirk-Grisell Funeral Home. Burial will follow in the Locust Grove Cemetery at Keavy.
The family will receive friends from 6-9 pm Tuesday at the Vankirk-Grisell Funeral Home.
